Community Case Management
Our Community Case Management services provide personalized support for individuals and families facing challenges such as housing instability, financial hardship, or health and mental health concerns. Our Case Managers work closely with clients to connect them with housing, financial assistance, healthcare resources, and mental health services, while providing advocacy and guidance to help navigate complex social systems.
Through programs like SOAR and broader community-based case management, we focus on fostering independence, resilience, and long-term stability. By collaborating with community partners and addressing both immediate needs and long-term wellness, we empower individuals and families to build safe, healthy, and supportive environments where they can thrive.

Community Case Management & Intervention
Day-to-day support, goal planning, advocacy, and crisis/safety planning with warm hand-offs to local resources.

Justice-Involved Case Management
Liaison with probation and parole, re-entry planning, appointment/reminder support, and connections to treatment, IDs, and work.

Housing & Tenancy Support Case Management
Housing search, applications, reasonable accommodations, tenant education, and landlord mediation to sustain stable tenancy.

Benefits & Financial Coordination
Navigation for SSA/Medicare/Medicaid, SNAP/LIHEAP and similar benefits, budgeting coaching, and coordination with Rep Payee services when applicable.
