
Community Choice Waiver
Wyoming’s CCW is a Medicaid home- and community-based program that helps eligible adults who meet a nursing-facility level of care receive long-term supports at home or in community settings. Through a person-centered plan, CCW can cover services like personal support, homemaker, adult day, respite, assisted living services, home modifications, meals, and transportation—so people can live safely and independently.

Case Management
In the Community Choices Waiver (CCW), BIAW provides person-centered case management that helps eligible adults live safely at home or in community settings. We handle intake support, assessments, and care planning; connect people to CCW services (e.g., personal support, homemaker, adult day, respite, ALF) and to State Plan/community resources; coordinate across providers; and monitor health, safety, and goals.

Homemaker Services
We help keep the home safe, clean, and functional so participants can focus on their health and goals. Staff provide light housekeeping, dishes and laundry, meal planning and simple meal prep, trash removal, shopping/errands, and basic home organization—plus friendly reminders for routines and appointments. Services are person-centered and scheduled at convenient times, aligned to the individual’s authorized plan. Homemaker does not include hands-on personal care or skilled nursing.
